That's my routine at Five Guys too, minus the cheese. And add lettuce ... because then I can say I ate a vegetable.

For a generic drive-thru burger: cheese, mustard, pickles

For a sit down place, I’m flexible with the type of cheese and sometimes subbing out A1 or barbecue sauce for the mustard. No mayo, no ketchup and except for pickles (and very occasionally mushrooms) no plant life on my burger.

Cheeseburger, ketchup only. Bacon is a plus option. If it somehow comes with lettuce, no problem, but won't ask for it.
Carl's Western Bacon Cheeseburger (or the double, which is No. 4 on your menu) is great, and the onion rings are easy to remove and give to my wife. No onions for me.

Can't go wrong with the basics -- ketchup, mustard, pickles, raw onions, lettuce and tomato. Maybe a little mayonnaise mixed in.
If I have my choice, though, then I'm more apt to go wild depending on how I'm feeling. BBQ sauce or A1 is good. Thousand Island dressing is great with the right mix of other vegetables on top. Grilled onions, mushrooms and jalapenos can add some nice change of pace. There are several different kinds of pickles (like Wickles vs. Bread and Butter chips) that can change things up a good bit, and the same with cheese.
I don't care for bacon myself, but don't begrudge those who do.

That's one of the awesome things about a hamburger, and what makes it an almost perfect food, is that it's so simple yet endlessly customizable. You can make a million different unique dishes just by tweaking what you put on it.

If a burger in a restaurant comes with bacon, I'll take it. I prefer it to be crispy.

At home, I'm too lazy to make a frying pan's worth of bacon when I'm grilling out or cooking burgers in a skillet. Grilled onions and jalapenos are doable, though.

Plant-based patty, leafy lettuce, a thick slice of tomato, red onion, dill pickle, and a schmear of ketchup and Just.

At home: sharp cheddar, onion, ketchup, AI, and when they're ripe local ones in summer, tomato.
Dining out: I have no set pattern except since I'm not cooking it, I try to have bacon on my cheeseburger (I haven't eaten a burger without cheese since I was like seven). There's a place near me that serves a blue cheese and bacon burger I enjoy. Also, maybe once or twice a year I'll have a patty melt.

Depends on my mood ... Burger must be ground chuck (80% lean, no more than that)

Bacon, blue cheese, lettuce, tomato and onion is excellent.

Burger, no cheese, sautéed onions, tomato and mustard is excellent.

Bacon, swiss cheese and mustard is excellent...

Sad that Catsup has been lost to the ages.
