IJAG, honestly, if you have health insurance, the ER's the way to go. Take a Russian novel, put a few old socks on your crippled foot and go to the ER. You'll get it professionally cleaned, mucho topical antibiotics, maybe even an Rx for a few oral antibiotics if necessary, and if they need to, they'll numb it and stitch you up. You use your foot too much to have it be fucked up longer than necessary; it's not like a cut on your left forearm or something. Put the old insurance to work.
