It implies that before, 20 million had no health care. Maybe they didn't have insurance, but they were typically cared for. Maybe they paid out of pocket (as I have done in the past). Maybe others were forced to pick up the tab.

Some may not have had insurance as a matter of choice. Some may not could have afforded it --- but then, some STILL cannot afford it but simply have to have it or get fined (that's really not a net positive).

Some countries run elections like that (vote for "X" or else), and we call them crooked. But we praise a "sign up for insurance or else" government policy. The fact that "20 million more have health care" is about as accurate as "candidate X got 95 percent of the vote" in my previous example.
